This is a large revolver with a float-shaped cylinder and a lanyard (string hole) visible at the grip end.
The TV version has the number 45 caliber engraved on the left side of the barrel, which is not present in the film version released in 2018, and Vash’s gun is also clearly marked as a large-caliber 45 caliber in the anime’s opening.
